BASHOF Grants

On September 11th The Bay Area Sports Hall of Fame presented a Youth Fund check
for $10,000 to the Wells Fargo Adopt A School Program here at our BASHOF offices.
BASHOF Alumni, Al Attles from the Golden State Warriors presented the check to the kids.
He was joined by Executives from Wells Fargo, KNBR Radio, as well as the Principals
and students from Roosevelt Middle School, Grattan Elementary School and Hamilton Elementary School.
Process:
Grant applications are available from September through January each year. Completed applications are due back
into our office by January 31 each year. Please complete all forms in their entirety and submit the indicated number of
copies, and

Make certain 2 of your 5 applications include the following supporting documents:
· Completed grant application (5)
· Athletic equipment request form (5)
· A written price quote from distributor, store or
a reputable sports equipment supplier (5)
· Revenue Summary (5)
· Schools must complete the BASHOF School Form (5)
· State and federal tax exemption letters (2)
· List of board of directors and officers (2)
· Current itemized budget (2)
· Financial statements of previous two years (2)
Two sets should be labeled: Chair and Investigator.
Please do not staple or bind the application and supporting documents.
Applicants will be notified of their grant status in July.
Criteria:
1. An organization must serve either the entire Bay
Area (i.e. the eleven counties of the Bay Area),
or must provide athletic activities for a
significant number of youths annually in at least
one Bay Area county.
2. An organization must be engaged in youth athletic
activities during non-school hours.
3. An organization must have a minimum three-year
history and must be a certified 501(c)(3)
organization (exempt from state and federal taxes).
4. An organization must provide an itemized annual
budget and financial statements to include projected
sources of funding and items of expense.
5. An organization must demonstrate the ability to
maintain adequate security for the equipment
purchased.
